Paperwork
for Athletes to Participate:
Any
student/athlete who is planning on participating in a Fall/Winter/Spring sport
for 2010/2011, he/she must have all the proper paper work filled out before the
first day of practice and/or tryouts. If he/she does not have all of them,
he/she will not be able to participate.
The paperwork includes:

Medical Form (2 pages)

Assumption
of Risk Form/Insurance Form (2 pages)

Form (for Drug Test)($27 fee)
