Immunization Alert for Middle School Students

New Immunization Requirement for 2010/2011 School Year


Effective with the 2010/2011 school year, one dose of the Tdap (Tetanus, Diphtheria, and Acellular Pertussis, adolescent and adult formulation) or Td (Tetanus and diphtheria, adult) vaccine shall be required prior to entry into the seventh grade. This dose is intended to be administered as a booster dose for students who have completed the required doses of the initial series of the DTap/DT/Td vaccine.


Please check with your physician or the Health Department (390-5600) to verify your child’s immunization status and schedule this booster if necessary. A copy of your child’s immunizations may be brought to school or faxed to Northridge Middle School (342-4631).

Permission to Administer Medication at School

To clarify the Northeastern Local School District policy about medication dispensing during school hours, written permission from a parent and from a physician is required for any medication taken by a student during the school day.  “ ‘Medication’ shall include all medicines including those prescribed by a licensed health professional authorized to prescribe drugs and any non-prescribed (over-the-counter) drugs, preparations, and/or remedies,” which includes Tylenol, Ibuprofen, and cough drops. A form giving your child permission to take medications may be signed by the parent and physician and returned to school (Click Here to print the form). For your convenience, the physician may fax the permission to 342-4631.
